Sci Rep 15, 32221 (2025).Roux-en-Y hepaticojejunostomy is a crucial procedure for treating biliary diseases, especially in patients with recurrent hepatolithiasis. However, the safety and efficacy of repeat laparoscopic Roux-en-Y hepaticojejunostomy(R-LRHJS) remain controversial due to the complexity of hepatobiliary stones and the potential for complications.In patients who have undergone multiple biliary surgeries, severe adhesions frequently develop in the right upper abdomen and around the liver, particularly at the hepatic hilum, as a result of tissue scarring and recurrent inflammation (Video 1). In some instances, incisional hernias of the abdominal wall may further complicate surgical procedures.Subsequently, cholangioscopy was utilized to remove all stones, and the stenotic segment was excised prior to performing a mucosa-to-mucosa interrupted eversion anastomosis. In cases of concomitant left intrahepatic bile duct stones, a hepatic resection of the affected segment of the left lobe was performed before proceeding with the hepaticojejunostomy (Video 2).
